Student Flu Vaccines
There will be two student flu
vaccination days scheduled for PES (3-5) students, depending on the type of
insurance coverage the student has.

Students who have
PEEHIP insurance will have flu vaccines available on November 4, 2015.
The forms for PEEHIP flu vaccines are not yet available, but will be sent out
as soon as possible.

Flu vaccines for all
other PES(3-5) students will be available during the school day on October
23, 2015. This day is for students who have any insurance other than
PEEHIP or students who have no medical insurance.

The flu vaccine form
for October 23 (non PEEHIP insurance) is attached. You may also pick up a
form from the clinic. Completed forms for the October 23 flu vaccine day
should be returned to your school nurse no later than October 16.

Please read over the
form carefully as the vaccine cannot be given if the form is not completely
filled out with all required information and signatures.

chronic health condition and has been advised by your physician to avoid
exposure to the flu mist or if you have other questions or concerns, please
